You WILL NOT be allowed on the base if you don't complete Form 5512. (When you sign Form 5512, Do Not Date It) Complete the Registration Form & Sub Tour Form; unless you are not interested in a tour of and Ohio Class boat.

Retirees with a valid Retired Military ID AND dependents with a valid Retired Military ID do not need to complete and submit form 5512.  Look at your Retired Military ID card, if it has your social security number on it, it is NOT valid.
